Louisville Metro Community Health Assessment

Louisville Metro Department of Public Health and Wellness (LMPHW), embarked on an initiative to achieve a greater understanding of the Louisville’s health. Using evidence-based methods, LMPHW conducted a Community Health Assessment (CHA). A CHA refers to a state, tribal, local, or territorial health assessment that identifies key health needs and issues through systematic comprehensive data collection and analysis.
